Liabilty Insurance Certificates

Liability Insurance Certificate must accompany application.  All vendors and organizations are required to have a Liability Insurance Certificate.  Please inform your insurance company to include "Forks Township Northampton County" and "Forks Community Days" as a Certificate Holder/ Additionally Insured on your insurance.  Also, Forks Community Days event dates must be noted. 

All vendors... food, businesses, and non-profit organizations... are REQUIRED to have General Liabilty Insurance in the minimum amount of $1,000,000 for each occurrence.  Both Forks Township Northampton County" and "Forks Community Days" must be named as Certificate Holder/ Insured.  Address 1700 Sullivan Trail, #149, Easton, PA 18040.
